Reentry Life Coach (Full Time)

Position Type: Grant-funded position contingent upon continued funding.

In the heart of Pomona and the San Gabriel Valley, Project Hope CA (PHCA) is rewriting the stories of justice-impacted individuals. What began as a grassroots initiative in 2018 is now a beacon of hope for people navigating the challenges of reentry after incarceration. As a 501(c)(3) nonprofit, PHCA supports justice-impacted individuals through transformative workforce development, life coaching (case management), and strength-based support services. We address the barriers that perpetuate recidivism and inequity, creating real opportunities for healing, growth, and economic stability.

Position Summary:

The RAY Program is a core part of LA County’s Department of Youth Development (DYD) mission to advance equity through prevention, early intervention, and reentry—rooted in youth development. As a grant-funded partner, Project Hope CA is seeking a passionate Life Coach (LC) to join our team and make a meaningful impact in the lives of justice-involved youth. Lived experience with the justice system is a valued asset.

As an LC, you’ll work directly with young people—building authentic relationships and helping them navigate challenges by providing care coordination, crisis response, system navigation, and conflict resolution. Through mentorship, youth will receive hands-on guidance and support from people who understand their challenges and are committed to walking alongside them.

What You’ll Do:

  • Mentor, motivate, and empower youth participants to become positive members of their communities by establishing a trusting and open relationship

  • Conduct outreach and engagement of eligible youth for program enrollment

  • Assess participants at the point of enrollment and provide appropriate linkages and referrals to needed services through an individualized care plan

  • Conduct intake and exit assessments

  • Manage a caseload of 10 youth participants. Maintain contact with youth participants 2 times a week at minimum, one of which should be in-person. Document interaction by updating case notes and care plans

  • Develop relationships with service providers to refer and link youth participants for any supportive services that are not offered in-house

  • Identify community resources appropriate to meet youth participants’ needs and assist through appointment accompaniment, transportation assistance, and coaching

  • Attend all required trainings and events organized by the County to foster learning and community

  • Exit youth participants who have successfully completed service and/or who are no longer actively engaged.

Qualifications:

  • Prior experience working with individuals with system involvement, mental illness, SUD, or experiencing homelessness, and/or experience assisting community members in navigating health and social service systems

  • Some experience with case notes documentation and/or case notes database system

  • Ability to communicate and build rapport with youth

  • Ability to follow program protocols and instructions

  • Ability to work independently and with a team

  • Ability to manage time and prioritize duties and time-sensitive tasks

  • Driver’s License, personal vehicle, and car insurance preferred

  • Bilingualism (Spanish) is beneficial

  • Basic computer skills

  • Lived experience with justice-system involvement is a plus!

Schedule:

  • Monday to Friday, 9AM – 5:30PM (with occasional weekends for special events)
